DescriptionPreparation and dispensing of drugs including IV medications and chemotherapy; maintenance and review of patient medication history and profiles; reviewing and verifying physician orders and patient medical records. Interacting with healthcare professionals and providers to help promote safe medication use, prevent inappropriate or misuse of medications; provide clinical pharmacy services (e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, TPN per pharmacy, clinical protocols per pharmacy, and pharmacists interventions. When assigned decentralized duties, the pharmacist will be physically located in the assigned area and primarily responsible for verification of medication histories/reconciliation. Other duties may be assigned. Schedule: Varied 8-hour shifts; depending on department needs including weekends and holidays Hourly Salary Rate: $97.38 (Standard Per Diem Rate)QualificationsRequired: CA Registered Pharmacist license Doctorate of Pharmacy degree from an ACPE accredited College of Pharmacy BLS and ACLS certification Preferred: Inpatient Pharmacy experience PGY 1 residency Decentralized experience
