In this key role, you will provide a wide range of services in support of multidisciplinary teams with an emphasis on outpatient practices of the UCLA-Community Physicians Network (UCLA-CPN). The Network currently serves adult and geriatric populations and may also serve child and adolescent populations in the future. You will be responsible for:

• Psychosocial assessment
• Care coordination
• Consultation to team
• Individual, family, or group psychotherapeutic treatment
• Liaison with community agencies
• Treatment plan documentation
• Disposition planning and advocacyQualificationsWe're seeking a self-motivated, compassionate individual with:
  • Appropriate California License and Degree (LMFT, Master's in Marriage & Family Therapy)
  • Experience interacting with health care providers in a multidisciplinary setting
  • In-depth knowledge of general theoretical and clinical models
  • Exceptional communication and interpersonal abilities
  • Extensive background in documenting/charting
  • Knowledge of social work ethics and scope of practice
  • Ability to perform a complete mental status exam and recognize changes in mental status
  • Knowledge of relevant community resources
