TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Santa Monica, California, 90404 About The Position Job Title: Registered Nurse - Case Manager Job Specialty: Case Manager Job Duration: 8-week assignment Shift: Day 5x8-Hour (08:03 - 17:00) Guaranteed Hours: Not specified Experience: Minimum 1 year required License: Valid State License required; license for specific state must be in hand Certifications: LA fire card within the last 30 days required Must-Have: - COVID-19 Vaccine: Required (Medical/Religious Exemptions and Declinations Allowed) - Ability to float between facilities as required for assignment - Commitment to work 3 of the 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day) - Availability to work either Saturday or Sunday, or both if able, with shifts from Tuesday to Saturday or Sunday to Thursday Job Description: - Responsible for admission criteria, including InterQual - Knowledge of drug/alcohol abuse treatment options - Understanding and managing cases involving patients experiencing homelessness - Experience assisting patients with mental health history or in crisis - Familiarity with advance directives and POLST - Knowledge of adoption and foster care processes - Ability to organize and participate in family meetings - Experience with HMOs and IPAs - Competency in arranging DME, SNF, and ambulance transport independently - Participation in the development of treatment plans - Understanding of protective services processes - Must have references covering 1 year of employment within the last 3 years All communication devices required to perform duties will be provided as necessary. Providers will be reimbursed for work-related cell phone use, up to a specified amount, if not provided directly.
