Location:14th St



Schedule:Monday- Friday 8am-4:30pm







JOB SUMMARY



With the support of and in partnership with the Assistant Director, Westside Outreach, the West LA Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals in high density areas of West LA. This team is part of a community effort by West LA Homeless to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services.The Case Manager is responsible for identifying, engaging and providing support to homeless clients, including providing referrals, linkages, and coordinating care. The Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.




ESSENTIAL DUTIES & RESPONSIBILITIES



1.Identify, engage, and provide support to clients in West LA Outreach program



2.Provide case management and aftercare/follow-up services to clients during their journey through recovery from homelessness



3.Report all statistics and updates for reporting purposes



4.Provide initialcontact, needs assessment, information, education, and referrals to the homeless



5.Provide case management, basic services, and advocacy for clients



6. Refer target clientele to appropriate services (e.g., health, mental health, interim housing, etc.)



7. Develop effective, trusting relationships with clients that facilitate provision of services



8.Work in collaboration with other regional outreach teams to connect clients to ensure continuity of care.



9.Interface and represent the agency with partners, including community health providers, business community, and social service/housing providers to reduce the number of homeless people living on the streets.
10.Maintain and record accurate electronic records, client files, and statistical information
11.Attend all case file, staff meetings, training, and retreats.QUALIFICATIONS



1.Bachelor's degree or equivalent experience



2.Demonstratedknowledge of and experience with the issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence)



3.Working knowledge of social services agencies on the Westside and LA county agencies serving the homeless



4.Excellent human relations/communication skills; ability to act in a professional manner regardless of circumstances.



5.Skilled in non-violent crisis intervention



6.Ability to self-motivate and organize time effectively



7.Computer literate; able to use computerized database systemfor information management.
